Medicare Part D Temporary ID Card

After you complete your Medicare Part D enrollment, you can print a temporary member ID card. You can use this card when your Express Scripts Medicare coverage begins (as early as January 1, 2021).

Your permanent ID card will be provided upon receipt of an approved application by the plan and the Centers for Medicare & Medicaid Services. You'll receive a letter of disapproval if your application is not approved.

Simply print your card and then follow these steps:

Fill in your name.

Contact Express Scripts Medicare to get the membership information needed to complete your card.

Call 1.866.477.5703, 24 hours a day, 7 days a week, except Thanksgiving and Christmas. TTY users, call 1.800.716.3231.

Use your temporary member ID card each time you fill a prescription until your permanent card arrives in the mail.

Express Scripts Medicare (PDP) is a prescription drug plan with a Medicare contract.
Enrollment in Express Scripts Medicare depends on contract renewal.
